We have a rare and exciting Job opening for a compassionate, inclusive Team Leader to join our CCN Team in Lincoln.

You will be responsible for the daily operational management and leadership of the team Monday - Friday 9am to 5pm providing direct nursing care, support and education to children and young people with acute nursing needs and those with long-term conditions and complex medical needs and their families.

Working flexibly you will meet the needs of individual children and families by offering 'before' and 'after' school visits as required.

You will work closely with the Grantham, Boston and Skegness CCN Teams to provide a county-wide telephone on-call service to the CCN caseload Monday to Friday 5pm to 10pm and at weekends 9am to 10pm, including participating in the 24/7 rota for children and young people requiring end of life care as required.

Applicants must be NMC Registered Nurse (Child) with experience as a Band 6 Community Childrens Nurse or experience of leading a children's community team.

An MSc / BSc(Hons) in Community Health with the Community Childrens Specialist Practitioner qualification or equivalent experience is essential. Qualifications inleadership or equivalent experience and teaching and mentoring are also essential for this role.

Main duties of the job

You will have responsibility to lead your team and provide high quality safe health care to children, young people 0-19 years and their families at home and in various community settings.

Working collaboratively across multi-agency / organisational boundaries you will initiate and coordinate appropriate services to facilitate the early discharge, prevent hospital admissions and manage children and young people with long-term conditions and complex care needs at home. You will have the confidence and ability to liaise with professionals within primary care and community health services, social care, education and voluntary services.

In partnership with individual children and young people and their families and all professionals involved in their care, you will formulate personalised holistic packages of care which incorporate the child / young persons feeling, wishes and goals (where possible) and promote the ethos of empowering children and their families to confidently self-care.

This will include preparing them for healthcare transition: working closely with the GP, District Nurses / Specialists, Consultants, Specialist Tertiary Centre and our Roald Dahl Transition Nurse Specialist to ensure young people have a positive experience in their journey across to adult services.

Qualifications

Essential


  • NMC Registered Nurse (Child)
  • MSc/BSc (Hons)Degree in Community Health with Specialist Practitioner qualification or equivalent experience
  • Teaching / Mentorship qualification
  • Leadership qualification or equivalent experience

Desirable


  • NMC recorded Independent / Supplementary V300 Prescribing qualification

Previous Experience

Essential


  • Experience as a Band 6 Community Childrens Nurse or experience of leading a community team
  • Experience of managing a nursing caseload and of working within the community setting
  • Evidence of a wide experience of children's nursing
  • Evidence of continued professional development and training
  • Experience of partnership working with internal and external stakeholders
  • Experience of child protection / safeguarding

Desirable


  • Quality Improvement experience
  • Previous experience in application of research in practice and audit

  • Ability to evidence / demonstrate key values and behaviours in line with the Trust Framework
