Integrated Community Services Patient Navigator

Working within speciality teams as an important member of the Integrated Community Care Group the Patient Navigators manage the service patient pathway taking into consideration all aspects of the service data management to include:
- Monitor reviews for the team both in Community venue settings and in the patient’s homes.
- Ensure all patients receive appointments in a timely fashion and any other aspect of managing the patient pathway.
- Interrogate data and support the planning of clinical staff and therapy colleagues workload.
We are looking for person with initiative who enjoys working with Teams and Excel.
Whilst the post holder will support teams Bay Wide, the base will be nearest to home with an option of blended working office/home to suit.
Interact with clinical and medical staff regarding referrals, out-patient appointments and concerns expressed by patients/relatives and General Practitioners. Communicate with staff as to the appropriate course of action, i.e. assessment or available clinic appointments.
Liaise and mediate between patients, relatives, and all clinical and medical
staff, exercising initiative and excellent interpersonal skills to ensure that such contact is sensitive and effective, showing empathy in distressing and emotional circumstances.
Also to act as a resource to the team therefore supporting appropriate management of patients within the community.
- To communicate with patients and professionals of all levels on a daily basis with regards to the patients’ journey.
- To engage with equipment providers regarding queries, orders and invoice discrepancies.
- To ensure the patient database is kept current and up to date and to ensure service compliance through the monitoring of patient appointments to the relevant clinics
- To have the ability to interrogate data to inform and enable planning of colleague’s workload and timely movement of patients through the various service pathways on a weekly basis
- To be able to prioritise workload and facilitate timely patient appointments in line with standards.
- To co-ordinate staff diaries and service delivery to accommodate meetings, booking and arranging meetings for the service.
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life-threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
