**Senior**
**Rough Sleeper Outreach Worker **to work as part of the new rough sleeper outreach service in Hull. The team will deliver an outreach offer alongside other partner agencies, ensuring that people that are rough sleeping are located and engaged with as soon as possible and referred into relevant services quickly which include Housing, Health, Benefits and Drug and Alcohol support etc.

**Salary**:£30,316 - £32,412

**Responsibilities**:
**About the role**:

- To coordinate a small team of Outreach Workers Day to ensure they locate, identify, engage, and support individuals that are rough sleeping, in the Hull area
- Going on outreach shifts; these include early mornings, nights, and weekends. The service covers 7 days and will operate 365 days per year
- May be required to provide one to ones with staff when management are not available to support
- Attend internal and external meetings ensuring that you are promoting the service as well as supporting within a Multi-Disciplinary Team model
- Ensuring that you provide a safe space for the people you will support, enabling them to express their thoughts

**About you**:

- Experience of engaging individuals with multiple support needs
- Able to work proactively and efficiently when engaging people and establishing support needs
- Relevant knowledge of at least one of the following: substance misuse, mental and physical health, housing and benefits, and the criminal justice system
- Experience of coordinating a small team
- To be person centred, and service user led in support planning
