The post holder will work as a member of the specialist multi-disciplinary team, carrying out assessments and providing effective treatment, care and support to clients who may be experiencing physical health problems in addition to their learning disability.

The role will involve completing assessments and risk assessments that inform clinical interventions and contribute to the planning of interventions and the completion of care plans. The role will also involve being the Care-Coordinator for patients with complex physical health needs that will include the monitoring and reviewing of patients physical health needs to achieve desirable clinical outcomes. The Post Holder will work in an empowering way and promote healthy living so patients can take responsibility for managing some of their health needs when this is possible to do so.

All aspects of the role require effective communication and collaborative working with partner agencies, GP's to plan and deliver LD Awareness training and promote the importance of making reasonable adjustments to ensure patients receive the best possible care. The strong links with GP's will involve providing support to identify patients who have a learning Disability and to verify the accuracy of the LD Register. The Post holder will work closely with our Band 7 Health Facilitator who will offer on-going support to prioritise the community and health facilitation elements of this role.

The Post Holder will also work in line with the Dynamic Support Decision Tool that identifies individuals that are at risk of premature mortality and work with patients who are assessed as being Red or Amber.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
