To support children and young people who require support from secondary mental health service - who are experiencing moderate - severe/enduring mental health difficulties. To work alongside the multidisciplinary team in delivering effective patient centred care to work collaboratively with children, young people, their families and other agencies to develop a collaborative plan of care and risk assessment. to utilise record outcome measures as part of therapeutic interventions, to record and measure progress. to offer an initial assessment and contribute to our initial assessment outcomes meeting to establish the needs of the young person to communicate effectively and work collaboratively with other pathways within the BEEU Service.
