**Job Purpose**: To lead, manage and oversee the co-ordination of local delivery of the Healthy Child Programme within the 0-19 integrated service requirements, using the healthy child programme as a framework to support delivery. This will include delivery and monitoring of delivery targets of all core Public Health commissioned interventions, including immunisations, health screening/surveillance, health assessments and a requirement to support the health needs of any vulnerable children/young people within the caseload. Work in collaboration with partnership agencies to address the needs of all children 0-19 years within vulnerable groups including those on the child protection register. To provide cover in conjunction with other members of the 0-19 Integrated team across Bridgewater Community Healthcare NHS Trust in times of annual leave, sickness and school holidays.

Duties and Responsibilities Management To develop caseload profiles, identifying priorities of need and facilitate in the planning of future service provision across the cluster areas. Using specialised knowledge and clinical expertise to ensure the needs of all children, young people and families are met, taking overall responsibility for the workload of the designated 0-19 team. Develop relationships to improve the quality of the service and co-ordinate clinical services across the children centres, schools, other educational settings, health centres, and GP practices and ensure any recommendations for action are implemented and regularly evaluated. Work with local public health analysts to identify population health needs for children and young people 0-19 years.

To be responsible for the integrated team members delivery of a universal service for children, Young people and families within a set locality, including vulnerable families in line with the Healthy Child Programme. Allocation and coordination of appropriately trained 0-19 integrated team members for High Schools and Clusters of Primary Schools and oversee the co-ordination of the School Health Plan with their named schools on an annual basis. Allocation of appropriate 0-19 team member to ensure representation on each children's centre advisory board. Allocations of integrated team members to act as a link to the 0-19 service via GP Practices, Childrens centres and early years settings Oversee the delivery of the recommended health assessment and reviews for children and young people.

Ensure comprehensive health assessments for identified children and young people, including annual health assessments for those in local authority care are carried out in line with the Healthy Child Programme meeting the needs of children and young people 0-19 years of age. To contribute to the coordination of the delivery of the national immunisation programme for school aged children. To contribute to the coordination of the Breastfeeding peer support service within a geographical area Provide guidance to plan, implement and evaluate interventions according to changing health needs to improve health outcomes. Advocate team to consult, advise and educate children, young people and families to enable them to effectively manage their health needs.

Work in partnership with other key stakeholders to safeguard and protect children and young people; Ensure collaborative working is promoted to support children and young people where there are identified health needs, or where they are in the child protection system, providing therapeutic public health interventions for the child and family Provide signposting to other services and agencies, referring and liaising appropriately. Take and act upon referrals from other agencies and services, as appropriate. Promote the 0-19 integrated team to be a confidential support and advice service that is easily accessible, for families, children and young people within all venues on a regular basis. Promote the organisation, facilitation and participation in health promotional and health educational activities in schools, childrens centres and community settings advocating collaborative working in partnership with all statutory and voluntary agencies.

Utilise opportunities to promote healthy lifestyles, positive mental health and the public health agenda. Ensure that children not in employment, education or training, or children educated at home receive the universal offer. Complete PDRs and return to work pro-forma as required to designated named staff within the team Ensure performance data is collated in a timely manner Oversee clinical audit within the team Lead on service improvement initiatives. Ensure service information is disseminated to all team members in a timely fashion Ensure appropriate delegation for all planned interventions within the 0-19 Team is in accordance with individuals skills and abilities.
