We are recruiting a Social Prescribing Link Worker for West Hill Health PCN located in the borough of Kensington and Chelsea.
Ideally we are looking to recruit a social prescriber who has already had some experience of working in this role.
You will have strong clinical support from all the practices who all highly value the support provided by our existing social prescribers. You will also have support from the management team of the PCN, and from the other social prescribers in the team. We have very strong links to our local community and high value the contribution that our social prescribers give to our patients and wider MDT team.
**Salary**: £27,000.00-£30,000.00 per year
Schedule:
- 8 hour shift
- Monday to Friday
Ability to commute/relocate:
- London, W2 5EH: reliably commute or plan to relocate before starting work (required)
**Education**:
- GCSE or equivalent (preferred)
