Charity People are proud to be partnering with Help on Your Doorstep, an Islington based charity which improves lives, enhances wellbeing and strengthens communities, to bring on board a Social Prescribing Link Worker.

The demand for GP appointments is high. However, a significant number of GP appointments are not directly related to medical conditions. Social Prescribing Link Workers work with GPs to support patients with non-medical issues by prescribing social activitiesand community-based initiatives, such as exercise groups, art classes or support groups, to improve health and wellbeing.

The Social Prescribing Link Worker will work with people with long term conditions (including support for mental health), who are lonely or isolated, or have complex social needs which affect their wellbeing. The postholder will make a visible differenceto the health and wellbeing of these communities by empowering individuals to overcome the barriers they face and improve their lives.

**Key responsibilities**:

- You will take referrals from GP practices, multi-disciplinary teams and provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health access and outcomes.
- You will take a holistic approached, co-producing simple personalised care and support plans to improve health and wellbeing by introducing or reconnecting people to local community groups and voluntary and statutory services and by referring individualsto partner organisations.
- You will manage and prioritise your own workload, follow up on all referrals and update records on all Help on Your Doorstep and NHS systems.

You will have experience of supporting people, their families, and carers in a related role within the voluntary and community sector. You will be an excellent communicator with an ability to empathise with people and provide person-centred support in anon-judgemental way, supporting people from all backgrounds and communities.
