We have an exciting opportunity for an enthusiastic Link Worker to join our dynamic and successful Enablement Service.

The Enablement service offers personalised support to address practical issues or resolve things that are getting in the way of a client’s health and wellbeing.

The Link Worker will work as part of a team, you will proactively work with clients to develop skills and maintain their independence, while reducing their risk of hospital re-admission or reliance/use of statutory services, specifically Adult Social Care. You will carry out outcome-focused support planning, focusing on the promotion of longer-term improved wellbeing and active engagement and connectedness with the community.

Enablement Workers will work across a range of settings including community, hospital, Adult Social Care, Client’s homes, to facilitate effective referrals and fast response times for clients.

This role is well suited to an individual with experience of directly working one-to-one with patients/clients, families and carers, using a strength based personalised approach to plan and help people to find solutions with meaningful long-term support that’s important to them. Based within Islington, you will be working closely with a range of professionals including Adult Social Care, Hospital Discharge Teams, Housing and Health practitioners, alongside other Age UK Islington services.

Your role involves working with people who might be experiencing complex issues, so you will need good listening and communication skills and be able to build effective partnerships with a wide range of organisations and community groups.

You will also manage a caseload, have case management responsibility and be responsible for forming effective partnerships with external agencies.

You will need excellent IT and organisational skills, be able to adapt to changing demands and priorities, be solution focussed, decisive whilst remaining service focussed. For more information see the full job description and Success Profile attached.
