**Great opportunities exist with a vacancies arising for Community Support Workers in the Wandsworth Recovery Support Team. Successful applicants will have scope to develop as professionals in the field of Mental Health, in a relevantly small multi-disciplinary team. This Team works with adults predominately aged 18-75 who have severe and enduring mental health conditions in the Borough of Richmond.**

**The successful applicants will assist the qualified clinicians in the team working as care coordinators, psychiatrists and psychologists to**:

- **Provide clearly defined recovery focused interventions, in line with patients' care plans.**:

- **Actively participate in the Team's meetings including Zoning, seeking to provide high-quality services strongly focused on recovery to patients**:

- **Positively promote and support independent living for patients in the community by maintaining extensive knowledge and links with the community resources and actively supporting patients to access them.**:

- **Enable patients acquire daily living skills: "living ordinary lives" (e.g. shopping and cooking).**:

- **Help patients gain access to resources and information about welfare benefits.**:

- **Help patients access physical health services and gain skills to monitor their health needs in the community on an on-going basis.
