Job summary

The Dementia Link Service provides targeted care and support for people with dementia and their carers, and provides a link between primary and secondary care teams.

The Dementia Link Worker will provide support and information for people with dementia living in the community in the London Borough of Hammersmith & Fulham, and their carers - linking them with appropriate social and health services.

The role involves on-going review and assessment, informing patients about dementia and liaising with GPs, the West London Health Trust and other services as required.

The post holder should have an ability to communicate effectively in writing and verbally. They should have previous experience of working with, and be empathetic to the needs of, people with dementia, older people or other vulnerable people in the community.

Ideally they should have competent assessment and care planning skills and knowledge of monitoring medication effects.

Main duties of the job

Main duties include but are not limited to:

assessment of individuals with dementia and their families Assessment and management of ongoing risks as identified during the assessment, ensuring that this is done in collaboration with service users and carers. Work as a member of an MDT. Deliver care in partnership with individual service users and their families and carers to ensure the delivery of the care plan. Monitor dementia medication, checking concordance and reviewing ongoing efficacy Acting as a link between service users, carers, GP, CIDs and any other relevant services Discussing cases of concern to CIDs MDT

Health Care or relevant NVQ Level 3 and / or equivalent skills acquired through experience.

Knowledge/Experience

Significant work experience in health/social care setting Experience of working with people with mental health difficulties / knowledge of local resources Knowledge of Mental Capacity Act (2005), to include practical knowledge of DOLS and Best Interest decisions Child and Adult safe guarding procedures.
