Fixed term for 8 months,starting on 1st July 2024.We have an exciting opportunity to join ourtalented team as a Dementia Adviser. You will have the rewardingexperience of providing information, guidance and support to peopleaffected by dementia in identifying their needs, to help maintainindependence, improving sense of well-being, and putting them in morecontrol of their lives.You will;Assist people with dementia and theircarers in identifying their needs, providing information and guidanceto help maintain their independence, improving their sense ofwell-being, and putting them in more control of their lives.Buildrelationships with a range of local contacts, networking with healthand care professionals, and providing dementia support.Developnetworks and pathways with partner organisations who work in the fieldof dementia, ensuring seamless and consistent support and empoweringindividuals affected by dementia to make informed choices.Providesupport face-to-face, by telephone and online and keeping detailed andconfidential records.

About youWe are looking for a highlymotivated individual, with an understanding of dementia, and the needsof those affected with these conditions. We are looking for someonewho is approachable and knowledgeable to join our knowledgeable andpassionate team. Our Dementia Adviser's support people in their homes.We interact face to face, over the telephone and virtually, to givepeople the tools and knowledge to make informed decisions about theirfuture. As a community-based worker, you will be required to attend arange of meetings, appointments and venues as well as working fromhome.You will;Be empathetic and non-judgmental in your approach with acommitment to equal opportunity.It is desirable that you haveknowledge and experience of Adult and Children Safeguarding,Information Governance policies and procedures and statutory laws suchas the Mental Capacity ActHave good communication skills to meet thediverse needs of our community and represent their needs to statutoryand other voluntary agencies.Have a sound knowledge of IT systems torecord data, write reports and communicate by email.Understand clientconfidentiality and how this is applied when representing clientneeds.Preferably had some experience of working with a wide variety ofrelevant agencies.Have the ability and means able to travelindependently around Somerset.
