We are hiring a Dementia Adviser to join the team at our Dementia Hub in Merton. If you’re looking to get experience in this area, or looking for a change, this might be the job for you.

Do you like working with people and really want to make a difference? We are looking for someone who can help people keep their independence, improve their sense of well-being, and put them in more control of their lives. As part of a commissioned service, we work alongside NHS and Local Authority professional staff, particularly in Primary Care.

People diagnosed with dementia and their families, friends and carers rely on us to provide guidance and training, helping them understand dementia, directing them to get the best support, and keeping in touch with them along the way. **About you**
- If you have a good understanding of the needs of people affected by dementia, this would be an advantage.
- You will need to work in an inclusive way, and understand the need for client confidentiality.
- You will manage referrals for clients who are newly diagnosed from the memory service and provide post diagnostic support.
- You will be speaking to people at all levels so great communication, networking and influencing skills are essential.
- You will be able to manage several tasks at once and be motivated to perform under pressure.
- You will have good IT skills and some experience working with databases.
- Be able to travel independently across London Borough of Merton.
