Job summary

West Merton PCN are looking for a qualified Pharmacist holding an independent prescribing qualification, who acts within their professional boundaries as part of a multidisciplinary team including, ANPs, GPs, Paramedics, Pharmacists etc and First contact practitioners.

Both Practices are located within a mile of each other, between Wimbeldon and Raynes Park and have good transport links and local amenitites.

Main duties of the job

As an independent prescribing Pharmacist you will be working alongside our wider PCN Pharmacist team of 3 Pharmacists, weekly team training led by our Senior Partner and Senior Pharmacist as well as support from both our clinical and non clinical teams.

You will help assist with the Practice Workflow and prescription management aswell as patient medication reviews, Structured medication reviews and drug monitoring.

About us

West Merton PCN is a busy, innovative PCN consisting of just two Practices covering a patient cohort of roughly 32,000 patients. The two Practices that make up West Merton PCN are Lambton Road Medical Practice (lead employer) and Francis Grove Surgery.

We do hold weekly education sessions for our Pharmacy team as well as support from our in house team daily.

Job responsibilities

Main responsibilities as a Prescribing Pharmacist:

Work as partof a multi-disciplinary team in a patient facing role to clinically assess andtreat patients using their expert knowledge of medicines for specific diseaseareas,beresponsible for the care management of patients with chronic diseases andundertake clinical medication reviews to proactively manage people with complexpolypharmacy, especially the elderly, people in care homes, those with multipleco-morbidities.

Provideleadership on person-centred medicines optimisation (including ensuringprescribers in the practice conserve antibiotics in line with localantimicrobial stewardship guidance) and quality improvement, whilstcontributing to the quality and outcomes framework and enhanced services

Throughstructured medication reviews, support patients to take their medications toget the best from them, reduce waste and promote self-care

Developrelationships and work closely with other pharmacy professionals across thewider health and social care system

Take acentral role in the clinical aspects of shared care protocols, clinicalresearch with medicines, liaison with specialist pharmacists (including mentalhealth and reduction of inappropriate antipsychotic use in people with learningdifficulties), liaison with community pharmacists and anticoagulation.

To act asthe point of contact for all medicine related matters, establishing positiveworking relationships and to consultpatients within defined levels of competence and independently prescribe acuteand repeat medication.

To receiveand resolve medicines queries from patients and other staff.

To manage atherapeutic drug monitoring system and the recall of patients taking high riskdrugs, , anticoagulants, anticonvulsants and DMARDs etc.

To providepharmaceutical consultations to patients with long term conditions as anintegral part of the multidisciplinary team

To provideadvice and answer medication related queries from patients and staff.

To improvethe quality and effectiveness of prescribing through clinical audit andeducation to improve performance against NICE standards and clinical andprescribing guidance.

To ensureappropriate supervision of safe storage, rotation and disposal of vaccines anddrugs. To apply infection-control measures within the practice according tolocal and national guidelines.

To supportclinicians with the management of patients suffering from drug and alcoholdependencies.

To activelysignpost patients to the correct healthcare professional

To handleprescription queries and requests directly

To participatein the management of patient complaints when requested to do so and participatein the identification of any necessary learning brought about through clinicalincidents and near-miss events

Toundertake all mandatory training and induction programmes

Tocontribute to and embrace the spectrum of clinical governance

Tocontribute to public health campaigns (, COVID-19 or flu clinics) throughadvice or direct care

Supportdelivery of QOF, NIS, incentive schemes, QIPP and other quality or costeffectiveness initiatives.

Improve thedata quality of medicines records and linking to conditions.

Delivertraining, mentoring and guidance to other clinicians and staff on medicineissues.

Working inpartnership with pharmacists and clinicians in hospital(s), improve the safetyand quality of prescribing after discharge from hospital admissions andattendance.

Provideleadership and support to prescription administrative/dispensary staff.

Person Specification

Qualifications

Essential

Must have completed their independent prescribing course. An understanding of working in Primary Care and it's prescribing needs. Previous experience as an Independent Prescriber.

Desirable

Experience in General Practice working as a pharmacist demonstrated within a practice portfolio Experience in managing pharmacy services in primary care In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
