The role is to provide a high standard of administrative support and ensuring a professional, efficient, and confidential service at all times for the Clinical Effectiveness Team and Learning from Deaths (LFD) portfolio. The post-holder will also play a key role in some important aspects of the Clinical Effectiveness department’s communications, activities and support selected projects.

To be responsible for supporting the Mortality and Learning from Deaths Team across the whole portfolio. Ensuring that information and/or assistance is provided to relevant staff as required, working closely with staff at all levels to support and promote real and lasting improvements in patient care.

To carry out day-to-day operation of the administration for the Mortality and Learning from Deaths portfolio and provide effective and efficient administrative support for the staff working in the service.

Ensuring that information and/or assistance is provided to relevant team members across the Mortality and Learning from Deaths portfolio.

Maintain the SJR (Structured Judgement Review) process and Mortality processes, which includes allocating SJRs (Structured Judgement Review) (Structured Judgement Review) to reviewers, processing completed reviews, capturing discussion points at Mortality Panel meetings, and ensuring learning points are received by clinical teams, Clinical Effectiveness Team, Patient safety team, LeDeR Team or other relevant stakeholders.

To maintain action trackers within the Learning from Death service.

Support Quality Initiative projects, and other areas as required, working closely with clinical staff at all levels.

Administer the Learning from Deaths Group producing high level minutes for dissemination.

Effectively respond to queries from the Medical Examiner, Coroner, LeDER, CDOP (and other external bodies) including providing request for clinical notes

Network and build professional relationships in acute hospitals and other NHS (National Health Service) organisations

Administer ad-hoc meetings as required by the Mortality & Learning from Deaths Manager/Specialist review as required.

Co-ordinate deep dives into themes identified through the SJR process.

To support mechanisms for implementing the presentation of valid, robust data which will support and inform clinical decision-making processes.

To assist in the assessment, production and dissemination of information supporting developments in line with Trust Clinical Governance arrangements.

To carry out day-to-day operation of the administration for the Mortality and Learning from Deaths Portfolio and provide effective and efficient administrative support for the staff working in the service

To support the administration of relevant software(s) used within service (primarily patient administration systems)

Collates data for audits and summarise findings appropriately for reports.

Work with the Clinical Effectiveness team and assist with the planning of the annual clinical Audit plan

Contributes to collation and analysis of data for designated Mortality and Learning from Deaths / CQC (Care Quality Commission) compliance.

Contributes to identifying risks in the workplace and to reporting of incidents, near misses and hazards.

Contributes as a team member to supporting other Clinical Effectiveness staff employed in the organisation to carry out the organisation’s Mortality and Learning from Deaths strategies.

Contributes to the development and documentation of systems and processes used by the staff working in the Clinical Effectiveness service.

Maintains and updates, as needed under the supervision of the Mortality & Learning from Deaths Manager/Project Manager, policy and procedure manuals, publications, reports, and other documents used by the Clinical Effectiveness service in relation to Mortality and Learning from Deaths.

Follows up as needed, to ensure that enquiries have been fully responded to and enquirers are satisfied with their interaction with the Mortality and Learning from Deaths service.

Disseminates correspondence, reports and other documents related to clinical effectiveness as needed, contacts clinical, managerial, and technical staff as needed to arrange meetings or participation in othe
