Planning and Organisational Skills Organize and prioritise own workload within agreed timescales building in flexibility to deal with unforeseen demands on time and resources Be familiar with the use of Electronic Patient Records (EPR) systems and any other computerized system integral to the running of the service Recognize the need for change and be proactive in delivering service improvements to both patients and staff Work closely with clinicians and admission managers to ensure that length of stay is appropriate Utilise advanced communication skills, including verbal and written when dealing with highly charged and complex situations Communicate sensitively and empathetically, diffusing situations of conflict when tensions arise around clinical priorities Work collaboratively with the patient flow manager to implement key divisional discharge process priorities, e.g. use of white boards, predicted discharge data Physical Skills During hours of working monitor that ward areas and equipment are kept clean and tidy to ensure the national cleaning standards are achieved and report any deficiencies to the ward manager Responsibility for Patient and Client Care Maintain currency with clinical developments, ensuring clinical understanding of these and their implications on the patient pathwayUtilise clinical expertise to advise on the movement and placement of patients, for example, according to clinical dependency and infection control requirements Challenge the multi-disciplinary team on the progression of treatment plans, addressing areas of concern and offering clinical advice where appropriate Assist with the development of appropriate audits in relation to bed capacity and the development of action plans in the light of findings Be aware of, and comply with, all Trust infection prevention and control policies, to include hand hygiene, personal hygiene, environmental and food hygiene Have an in depth knowledge of the physiological, pathological, social and cultural issues which influence the care of patients Participate in local audits and quality ward rounds as appropriate Maintain appropriate records and be able to demonstrate that all documentation adheres to Nursing & Midwifery Council (NMC) and Trust guidelines and that patient confidentiality is maintained at all times, in line with Data Protection Act Demonstrate a working knowledge of Trust Policies and guidelines and protocols Ensure that all annual mandatory training is completed and other relevant courses are attended in line with Trust and local policies To have knowledge of the Mental Capacity Act (2005) and The Care Act (2014) with an understanding of how this impacts of the discharge of patients from an acute setting Responsible for policy implementation and for discrete policy or service development for a service or more than one area of activity Undertake annual discharge and transfer audits with support of manager Train staff as necessary in relevant knowledge area Ensure that resources are used efficiently and effectively to ensure value for money Ensure that equipment is used appropriately and report any non-functioning equipment Reduce extended LOS across the inpatient wards Responsibility for Human Resources Act in accordance with the NMC Standards of conduct, performance and ethics, and to be accountable for own clinical practice and professional actions and omissions at all times Ensure continued and effective registration with the NMC Delegate to suitably qualified and experienced members of the ward team as appropriate Act as mentor to all staff; provide clinical support, professional advice, consultation and direct assistance to colleagues as necessary Assessment and mentorship of pre and post registration student nurses Responsibility for Information Resources Ensure that patient records are maintained, and act as a key worker with the multi-disciplinary team Maintain patient confidentiality at all times Responsibility for Research and Development Identify, collect and evaluate information to develop specialist knowledge of work procedures and practices underpinned by theoretical knowledge Take an active role in contributing towards multidisciplinary clinical audit Responsible for data entry, text processing or storage of data, utilising paper or computer based data entry systems
