We are looking to recruit Band 3 nurses on a full time basis within the team. The Working hours are 8-4 Mon to Friday

We are looking for an innovative, motivated and enthusiastic professional to join our ever growing team We are seeking a proactive team player who can join our diverse and dedicated team of Complex Discharge Planners

The role involves working as part of the multi-disciplinary team to maintain and expedite patient flow through the wards with the support of Complex Discharge Nurses.

Liverpool University Hospitals NHS Foundation Trust was created on 1 October 2019 following the merger of two adult acute Trusts, Aintree University Hospital NHS Foundation Trust and the Royal Liverpool and Broadgreen University Hospitals NHS Trust.

The merger provides an opportunity to reconfigure services in a way that provides the best healthcare services to the city and improves the quality of care and health outcomes that patients experience.

The Trust runs Aintree University Hospital, Broadgreen Hospital, Liverpool University Dental Hospital and the Royal Liverpool University Hospital.

It serves a core population of around 630,000 people across Merseyside as well as providing a range of highly specialist services to a catchment area of more than two million people in the North West region and beyond.

To provide a single point of contact on a named ward/wards for patients, families, carers and associated people and co-ordinate/contribute to the safe and timely hospital discharge in partnership with other multi-disciplinary colleagues.

Ensure that patient and family/carer have clear expectations regarding anticipated length of stay.

Maintain momentum of discharge planning throughout the entire process; supporting and working in partnership with other members of the MDT; doctors, Nurses, occupational therapists, physiotherapists and other hospital assessment and discharge team members.

Establish and maintain productive integrated working relationships with social service and community healthcare colleagues as required.

To develop strong working relationships with ward teams, Discharge Lounge, Therapy teams, Discharge co-coordinators pharmacy etc., to support and co-ordinate timely and appropriate assessment and safe discharge of people from acute beds.
