Lead Discharge Co-ordinator

The Surgical Pathways Manager will be expected to participate in the strategic development of the Surgical divisions service by:
- being recognised as an expert in their clinical field
- holding specialist knowledge and experience
- providing leadership in clinical practice for the professional team and peers
- ensuring an effective learning environment for students and peers
- working as a competent, autonomous practitioner / specialist
- actively disseminating best practice through teaching, and presentations and by participating in nursing / allied health professional research
- To be a key management role at a senior level within the Surgical Directorate, providing specialist knowledge and skills to ensure the delivery of care by maximizing patient flow through the surgical service
- To lead on the coordination of admissions and discharges within the division in order to ensure maximum utilization of Trust resources and to ensure a good patient experience
- To identify barriers to optimal patient journeys and system flow, acting in partnership with senior colleagues to resolve issues and make effective use of surgical bed stock, ensuring that national and local targets are achieved whilst maintaining the safe and appropriate in-patient placement of both elective and emergency surgical patient admissions.
- To be responsible for the leadership and line management of discharge support workers within the Surgical Directorate and to maintain close links with the Neurosciences discharge and senior team.
- To develop strong working relationships with colleagues in bed management, site management, patient pathway administration, 18-week team, service and general management, matrons, ward managers and SNCT senior divisional team.
- To ensure policies pertaining to the safe discharge and repatriation of patients are upheld.

St George’s University Hospitals NHS Foundation Trust serves a population of 1.3 million across southwest London. A large number of services, such as cardiothoracic medicine and surgery, neurosciences and renal transplantation, also cover significant populations from Surrey and Sussex, totalling around 3.5 million people.
The trust also provides care for patients from a larger catchment area in southeast England, for specialties such as complex pelvic trauma. Other services treat patients from all over the country, such as family HIV care and bone marrow transplantation for non-cancer diseases. The trust also provides a nationwide state-of-the-art endoscopy training centre.
