Pathway Co-ordinator

The successful post holder will be primarily responsible for co-ordinating the Rapid Access Pathway for patients referred into the service, under the 2 Week Wait rule, with suspected bladder, renal and testicular cancer. The pathway has been designed as a 'one-stop' service to ensure patients are either rapidly diagnosed or discharged.
The Rapid Access Haematuria Clinic (RAC-H) is run daily as part of the urological cancer service offering, with all diagnostic investigations taking place on the same day. Typically, this will involve a CT urogram or ultrasound scan followed by a flexible cystoscopy, which is the key diagnostic tool for bladder cancer.
- Co-ordinating the rapid access haematuria clinics, ensuring that clinics are booked appropriately and that patients receive relevant information prior to their appointment
- Monitoring urothelial and renal cancer Patient Tracking Lists and liaising with the bladder and renal cancer MDT Co-ordinators and other key stakeholders to ensure patients are seen in line with clinical urgency and Cancer Waiting Time Standards
- Liaising with General Practice and other NHS Trusts to ensure that the relevent information is available to to the clinical team to ensure continuity of care for patients
- Providing administrative support to the wider multidisciplinary team, including medical, nursing and administrative colleagues
- Attending weekly Post MDT Wash-Up meetings, in the absence of the Pathway Co-ordinator for Bladder and Renal Cancer Outpatient Clinics, and completing actions regarding the care of patients on their cancer treatment pathway
