Pathway Co-ordinator

The successful post holder will be primarily responsible for co-ordinating the functional outpatient clinics to ensure patients are seen in line with clinical urgency and the waiting time. Functional urology is an area of expected growth within ICHT Urology. This is a strategically important area for the Urology department. This post holder will support the service in managing this growth successfully, ensuring good patient experience and efficient pathway management are maintained.
The post holder will also be expected to provide support, where necessary, to cross cover other Pathway Co-ordinators within the Imperial benign urology team.
Monitoring the Patient Tracking Lists and liaising with key internal stakeholders to ensure patients are seen in line with clinical urgency and RTT Waiting Time Standards
Liaising with General Practice and other NHS Trusts to ensure that the relevent information is available to to the clinical team to ensure continuity of care for patients
Providing administrative support to the wider multidisciplinary team, including medical, nursing and administrative colleagues
Attending weekly MDT meetings, in the absence of the Pathway Co-ordinator for Andrology, Stones and Lower urinary tract symptoms (LUTS) Outpatient Clinics, and completing actions regarding the care of patients treatment pathway
