Please see attached Job description for full details To make a valued contribution to providing a quality service for patient/clients through the delivery of an exercise or rehabilitation programme in various locations across the city, virtually or directly to people in their own homes. Contribute and advise where applicable in the delivery of healthy lifestyle and physical activity education programmes, where appropriate, specific to their diagnosis To implement a range of rehabilitative, exercise programmes for specific patients/clients, in a gym, patients own home or virtually To evaluate the progress of a patient/client under their care and provide condition specific advice and education in a number of settings including group education. To deliver a range of high quality physical activity opportunities which are progressive, graded and reflect the needs of those referred to the service. To actively promote the health related benefits of physical activity to patients/clients.
To ensure all health and safety procedures are adhered to, and to maintain patient/client confidentiality in line with trust policies. To carry out safe inductions, complete client personal programmes, demonstrate equipment and individual goal setting. To complete clients programme reviews as specified. To actively encourage individuals who have completed a supported exercise programme to continue to participate in physical activity.
To work in collaboration with other members of the MDT to deliver a condition specific rehabilitation programme with individuals with low, moderate and high acuity for exercise, working within the sphere of responsibility defined by the BCAPR qualification and level 4 pulmonary rehabilitation. To work as part of the integrated team to support the service with the aim of returning to maximum independent living, promoting self
- management of activities of daily living. To deliver programmes of cardiac and pulmonary rehabilitation in collaboration withan Allied health professional. Use motivational interviewing and behaviour changes techniques to work collaboratively with patients to support self motivation and involvement in their own care.
