**Background and context**
The Lambeth GP Federation is responsible for the delivery of the Enhanced Access Hubs in Lambeth. The Lambeth GP Federation is commissioned by the PCNs and currently holds an SLA with host practices to provide 5 geographically based Enhanced Access Hubs, which provide extended primary care access for patients registered across the Borough.
**Job Purpose/Summary**
- The post holder will be responsible for ensuring that each Enhanced Access Hub is fully functioning, which includes setting up, and filling rotas for Clinical and Non-Clinical staff.
- The post holder will have responsibility for the continued utilisation of the hubs across the borough.
- The post holder will work closely with the Medical Director and the Senior Services Manager to ensure that the services provided meet the aims and objectives as outlined in the service specification.
- In conjunction with the designated leads, the post holder will support the phases of development as it relates to our Business Plan.
**Key Responsibilities**:
Service Management:
The post holder will:
- Utilise Rota Master to set up robust rotas across 5 Enhanced Access Hubs
- Ensure the rotas for each Access Hub are filled on a week-by-week basis
- Work with the host practices to oversee the day-to-day management and smooth running of the hubs
- Support the implementation of new policies and directives as required
- Prepare data for the monthly Operations Board and Integrated Governance Committee Meetings
- Gather and analyse relevant data and use it to monitor and plan the service
- Work towards ensuring quality and value for money for patients
- Plan and implement changes to improve service delivery
- Attend meetings, produce reports, and prepare presentations for a variety of audiences including PCN colleagues and relevant commissioners
- Ensure clinical governance is adhered to and record any Significant Events
- Undertake regular audits, with a view to presenting these to relevant colleagues at Board and Core Team meetings
- Work with the host practices to ensure the service premises are suitable
- Work with the host practice to ensure a sufficient level of equipment and supplies are consistently available
- Provide support for IT systems and technology solutions
- Maintain good relationships with Locum Agencies, ensuring a good calibre of staff is consistently available
- Liaise with the weekend on-call Senior Service Manager regarding covered shifts, contact details and possible issues
- Work closely with the Senior Service Manager and Medical Director regarding complaints, Serious Incidents and Near Misses
- There will be some on-call responsibility as part of this role
- Achieve and demonstrate agreed standards of personal and professional development within agreed timescales
- Undertake any other duties at the request of the line manager which are commensurate with the role, including project work

- Attend meetings, forums, working groups to represent the service, on behalf of the team or Lambeth GP Federation as appropriate
