An exciting and challenging opportunity has arisen for a 6 month fixed term contract within the Integrated Independence Team for an innovative, enthusiastic and experienced Physiotherapist or Occupational Therapist to be a Clinical Co-ordinator within the Single Point of Access.

We are looking for an experienced band 6 or existing band 7 Physiotherapist or Occupational Therapist to coordinate the initial screening of all referrals into the service, as part of the single point of access team. You will also help provide a high level assessment and advice service for complex patients in the community. There is also the opportunity to be a part of the weekend IIT rapid response team working in the Emergency Department and service users homes. In doing this you will work as part of a strong multidisciplinary team, within an interdisciplinary working model, to facilitate hospital discharge and prevent acute admission.

The main duties of the
- Together with the team of Clinical Co-ordinators, coordinate the IIT Single Point of Access, screening all referrals and allocating cases to appropriate care pathways
- Liaising with primary, secondary, health and social care providers, to case manage individuals into appropriate service or declining referrals, where necessary
- Promote independence for service users, enabling them to live in the least restrictive environment whilst ensuring they receive the necessary care and treatment
- In-reach to acute hospitals
- Assess clients referred, when required, to decide on appropriateness for the service
