We have an exciting opportunity for an NMC registered Band 6 Personalised Care Co-ordinator to join our dedicated Cancer nursing team, to support with personalised care delivery across all cancer pathways,
The post holder will work collaboratively with the Project manager for Personalised care & Optimal Pathways, Lead Cancer Nurse, Cancer Information and Support Managers and clinical teams to ensure that personalised care is delivered across all cancer pathways. The post holder will be required to organise and support with Trust Cancer Wellbeing events, education, health promotion and developing links with wider community services.
- The post holder will work collaboratively with the Project Manager for Personalised Care & Optimal Pathways and will support the implementation and delivery of the Personalised Care agenda across the cancer pathways within the Trust.
- The post holder will be required to work collaboratively with the clinical teams delivering Cancer Care, and the cancer management team to support the delivery of high-quality care.
- Work in collaboration with the Macmillan Cancer Information and Support Managers to deliver personalised care within the Trust.
- The post holder will be required to ‘navigate’ and undertake the Holistic Needs Assessments (HNA’s) of patients and develop a care plan in conjunction with the patient to support the delivery of interventions to meet the needs identified, including where appropriate signposting to other services.
- Maintain and develop links with organisations that can assist patients with vocational rehabilitation and broader health care related issues.
- Undertake relevant training, to deliver and facilitate the HOPE self-management programme and to work in partnership with the Macmillan Information & Support Manager to organise regular generic and site-specific Wellbeing events.
- Support the clinical team with education events, health promotion and cancer awareness.
The Queen Elizabeth Hospital (QEH) King’s Lynn is located near some of the most beautiful scenery in the UK, along the north Norfolk coast, and not far from Sandringham House.
We provide a comprehensive range of specialist, acute, obstetrics and community-based healthcare services to around 331,000 people across west and north Norfolk, in addition to parts of Breckland, Cambridgeshire and South Lincolnshire.
We have more than 4,000 staff and volunteers, approximately 530 beds, and a helipad for air ambulances. We work with neighbouring hospitals for the provision of tertiary services, including as part of regional partnership and network models of care, such as the trauma network.
In February 2022 the significant progress that has been made at QEH was recognised by the Care Quality Commission who rated the Trust as ‘Good’ in all of the core services they inspected. They recommended the Trust moves out of the recovery support system (formerly special measures).
