Job summary

Wells Medical Centre and Burnham Surgery, part of Coastal Primary Care Network (PCN) in West Norfolk are seeking to recruit a care coordinator on a part-time basis for a minimum of 25 hours per week.

Working alongside the practice manager and administration team, the care coordinator will provide administrative support and carry out healthcare tasks to meet the needs of our patients.

Main duties of the job

Act as a point of contact between the practice, patients and carers and other agencies. Liaise with HCAs, nurses, GPs, and practice teams to identify patients who have long term health needs and support. Complete and action tasks appropriately for the administration team. Support the practice with administrative tasks associated with patient care. Patient data collection through the registration process. Be involved and participate in consultations to help support the care arrangements for patients with long term conditions. Undertake telephone contact to manage patients on the practices case load following any unplanned hospital admissions where appropriate. Raise awareness of health promotion, screening and NHS health checks and LD health checks and invite patients for these reviews. Co-ordinate practice clinics, and contact identified patients with appointments. Manage patient-initiated calls for help/signposting etc., ensuring patients are directed to appropriate service.

Wells Health Centre is a small dispensing practice on the coast rated as Outstanding by the CQC.Burnham Surgery Burnham Surgery is a small GP Practice on the Norfolk coast rated as Good by the Care Quality Commission (CQC).It provides care to around 3281 patients in and around Wells in North Norfolk. In January 2022 the Burnham Market are joined with Wells with a patient list size of 4300.

Essential

Experience or evidenced understanding of general practice. High levels of organisational skills, being able to support fast paced working and decision making. An understanding, acceptance, and adherence to the need for strict confidentiality. High level of verbal, written and numerical literacy. Strong communication skills, with both practices, patients, and partner organisations. Ability to use own judgement, resourcefulness, and common sense. Good IT skills.
