This Community Discharge Worker role is based within our specialist discharge service, and you will be working within a multi-disciplinary team to support individuals in their first six weeks post-discharge from hospital.
This role is fast-paced, interesting, and always rewarding
You will be working with a variety of individuals who may experience a range of diagnoses and needs; however, our goal is to improve patient experience and empower individuals to regain their independence outside of a hospital setting.
We have full, part-time, and flexible roles available.
Your role as a Community Discharge Worker will be delivering quality support that enables people to achieve their outcomes by using and developing person-centred support plans. You will be able to manage risks and vulnerabilities, whilst promoting socialinclusion, and maximising the opportunities for the people you support.

**Essential Skills** To be successful as a Community Discharge Worker you should have:
- an enthusiasm and interest in working within the mental health sector
- excellent verbal and written communication skills
- a willingness to go the extra mile
- a flexible approach that respects individual differences
- an ability to work as part of a team as well as independently, using your initiative
The role will be working with individuals being discharged from inpatient settings across the county and so the role involves supporting those across the West Sussex County.
**Desirable Skills** It would be desirable for you to have had experience of supporting a person(s) with a mental health condition and within a community-based services.
