Guild Care is seeking a dedicated Take Home and Settle Support Worker to join our esteemed Community Services team If you're ready to make a difference by ensuring a smooth transition for discharged hospital patients while promoting their safety and wellbeing, then this role is perfect for you.
- **Location**:Worthing, West Sussex
- **Salary**: £11.93 per hour
- **Hours**:14 hours per week

**Essential Requirements**:

- Bring your experience of working with vulnerable adults to the table
- Be available every other weekend, including Saturdays and Sundays
- Possess knowledge of health and prevention issues for older people
- Understand the value of low-level intervention in promoting wellbeing
- Ability to provide a good standard of customer service
- Communicate appropriately and effectively while maintaining confidentiality and managing boundaries
- Possess a current driver's licence and access to a car

**Desirable Skills**:

- Ability to work well as part of a team with internal and external agencies and partners
- Familiarity with Primary Care, Social Care, and Voluntary Sector services
- Knowledge of Community Services in the Worthing and Adur area
- Health and Safety qualification
- Qualification in care
- First Aid qualification

**Your Role**:
As the Take Home and Settle Support Worker, you'll play a pivotal role in ensuring the smooth transition of discharged hospital patients back to their homes. Here's what you'll be doing:
**Person-Centred Support**:

- Ensure that the customer remains at the heart of all the work undertaken
- Respond to concerns about the physical or emotional health of customers, gaining their trust to identify their needs effectively

**Information and Referral**:

- Provide relevant information and advice, assisting individuals in accessing voluntary, statutory, and benefits services to improve their quality of life
- Encourage individuals to look after themselves, supporting them in changing behavior and lifestyle to enhance health and wellbeing

**Assessment and Reporting**:

- Conduct assessments consistently and record outcomes of visits accurately
- Report any major concerns immediately to the line manager

**Collaboration and Networking**:

- Liaise directly with professional colleagues, local service providers, family, and friends, ensuring close communication and integrated working on behalf of service users
- Share information proactively when appropriate, maintaining appropriate levels of confidentiality
