B7 Paediatric Sickle Cell CNS 0.This posts will be based within a well-established nursing team working across Southwark, Lambeth, and Lewisham.


Our CNS team work with a diverse patient demographic providinga range of support including antenatal support, new-born visits, creation and reviews of care plans, support for patients discharged from hospital after admission.


The post holders will lead and manage nurses to develop and deliver personalised care packages for individuals with sickle cell and thalassemic conditions.


You will manage a caseload of patients, ensuring each patient has an up to date care plan, conduct home visits, and link in with other care providers to provide wrap around support for patients and ensuring their safety and well being.

1 Professional/ Clinical responsibilities To provide a comprehensive genetic counselling service that covers opportunistic,antenatal and neonatal counselling/ screening in line with the national screening programmes. To work autonomously, managing a caseload of patients within the specialty from 0 to 18yrs, delivering individualised and personalised direct patient care with a case management and disease management approach. Work in collaboration with the transition nurse / team with young people around their transition from children to adult services. Provide care in acute and chronic pain management as per agreed protocols Recognise and co-ordinate support for other clinical or social needs in families e.g. mothers presenting with post-natal depression. Collaborate with Social Services in providing support to vulnerable children and families diagnosed with sickle cell disease and thalassaemia major. Make referrals, carry out assessments and prepare reports for Case Conferences in collaboration with Safe-guarding Teams as appropriate Provide support for and refer families with emotional and mental health needs e.g. bereavement counselling. Assess the need for welfare benefits/ housing/ education employment support and refer to the team Welfare Support Advisor as required.5 To act as advocate for patients within the area, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines. To ensure that patients receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines. Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals. Carry out those activities necessary to conduct a comprehensive assessment of a persons nursing requirements. Carry out nursing procedures and treatments, checking on and maintaining the highest possible standards. Work alongside the multi-disciplinary team to ascertain treatments and advise on nursing priorities. Take a clear and concise history from patients by assessing their health and well being and complete documentation ensuring that all entries are accurate and legible and that all information systems are maintained. Participate in the promotion of patient centred care, and establish and maintain a supportive relationship with the patient and their family during the period of assessment, treatment and after care Support and enable patients and carers to make informed decisions relating to their treatment and management. Promote high quality clinical care, and assist in the development of the specialist service, through the use of evidence based practice and clinical standards Provide professional and clinical leadership and expertise in the speciality and to act as a resource for education and clinical expertise in the hospital and community settings. Demonstrate appropriate expert practice and highly specialist advice in theassessment, planning, implementation and evaluation of patient care.2 Quality6 Facilitate a high quality, cost effective specialist service through monitoring and audit and other quality initiatives agreed by the Trust Lead on and participate in key developments relating to the specialty so that high quality patient focused services are promoted. Enhance the interface between acute/ community staff and patients. Set, monitor and review standards of care on a regular basis and initiate corrective action plans Identify and intervene where circumstances contribute to an unsafe environment for patients and staff. 3 Education and Training Act as a facilitator/mentor/preceptor to student nurses and others. Assess own educational needs and take steps to keep up to date with clinical and nursing developments in line with requirements of the Nursing & Midwifery Council.. Develop the partnership between the Trust and academic staff at local Universities involved in pre-registration and post-registration nursing and medical programmes. Ensure that the learning environment and specialist training is appropriate for both pre and post registration learners and participate in the development of appropriate programmes Work closely with the Clinical Nurse Manager, ensuring that relevant clinical based teaching programmes are in place for all staff Contribute in the Trusts clinical supervision, and participate in performance management in keeping with organisational structure. Lead and participate in education and training programmes within the specialty service, and develop training packages as required. Co-ordinate the development and delivery of a patient focused education and information giving strategy within the specialty7 Motivate, develop, support and identify training needs for department staff. Ensure that appropriate written induction programmes are available and utilised for all new staff within the department Be aware of the local equipment training programme for staff in relation to the Clinical Negligence Scheme for Trusts and maintain training records.Utilise research findings in the delivery of specialist patient care, keeping other staff informed of new research information in nursing and in the field of haemoglobinopathies. Conduct peer audits to highlight effectiveness of service, identifying gaps and propose improvements to maintain high quality service. To be familiar with current clinical research protocol issues and relevant treatment and procedures employed in the diagnosis and treatment within their specialty.5 Management and leadership Provide line management of the specialist nurses within the service. Maintain accurate records of the clinical service and ensuring that confidentiality of information is adhered to Organise own work and manage own caseload and practice, identifying and maintaining supportive networks for self and other staff members.8 Implement new initiatives where possible to run a cost effective, timely service To be involved in the development, assessment and implementation of guidelines, protocols and pathways. Implement and work within the Trusts HR policies on all staffing matters, including sickness, capability and disciplinary issues. Adhere to Trust and departmental policies and procedures, ensuring that all staff do the same.6 General Act in accordance with the Nursing & Midwifery Council Code of Professional Conduct and Scope of Professional Practice and be accountable for his/her actions at all times.
