Job Title: Head of Community Specialist Nursing

Job Summary:

We are seeking an experienced and skilled professional to lead our Community Specialist Nursing team. As Head of Community Specialist Nursing, you will be responsible for developing and implementing evidence-based clinical nursing practice, leading the team, and providing expert specialist nursing services to individuals with specialist needs.

Key Responsibilities:

  • Develop and implement evidence-based clinical nursing practice in line with current research and guidelines.
  • Lead and work with the Community Specialist Nursing team to provide high-quality nursing care.
  • Establish clear lines of communication with primary healthcare teams to increase knowledge and facilitate care in the community.
  • Provide and manage an expert specialist nursing service for individuals with specialist needs.
  • Work with the Multi-Disciplinary Team (MDT) to develop, implement, and evaluate integrated care pathways and systems of MDT documentation.
  • Liaise with local community, social services, paramedics, and voluntary organizations to develop existing services for clients, carers, and health professionals.

Requirements:

  • Advanced communication skills.
  • Understanding of Personalised Care National Agenda.
  • Publication in healthcare journals and presentations at conferences.
