We have an exciting and rewarding opportunity available for an on-site Dementia Care Navigator to join our team at Broughton House Veteran Care Village in Salford Greater Manchester, engaging with the local veteran community, until 30th April 2025.

The AFCFT has awarded us a one-year grant to employ a part-time Dementia Care Navigator for 12 months to help Armed Forces veterans with dementia living in the local community and their carers access the help and support they need.

Detailed project description

We'll employ a Dementia Care Navigator for 18-hours-a-week specifically to support veterans with dementia living in the community in Greater Manchester to help them and their carers access the community-based support they need. The DCN will also support veterans who are carers of someone with dementia.

Responsibilities:

Over a 1-year trial, the DCN will directly support 40 veterans with dementia and 20 carers. They will:

· Provide a single point of contact/expertise for veterans with dementia/carers to understand and then access community-based support available to them, including providing expertise in providers' commitments to support the Armed Forces Community and their rights under the Community Care Act.

· Meet veterans with dementia/carers face-to-face to undertake a needs-based assessment to understand the help they need and the challenges they currently face in accessing effective support in the community.

· Help the veteran/carer develop a 'needs-led action plan', identifying areas where they need support and the outcomes they wish to achieve to improve their quality-of-life.

· Using their specialist knowledge of the health and social care system, help the veteran/carer navigate through the available support so they can effectively implement their action plan. This might include helping them access benefits (e.g., Attendance Allowance/Carers Allowance); practical help (e.g., home adaptations/personal care); specialist statutory/voluntary dementia services (e.g., Memory Clinic; our own veteran-focussed Admiral Nurse, help from Alzheimer's Society); respite support; and access community/voluntary sector services and activities to reduce isolation for both the veteran and the carer.

· Over a period typically lasting 3 months provide tailored, 1-2-1 support and advocacy to ensure the veteran/carer implements their plan and receives the holistic, person-centred support they need and are entitled to.

The DCN will work closely with and under the guidance of our Admiral Nurse, who provides specialist dementia care to veterans living at Broughton House, helping bring our expertise in dementia care to benefit veterans in the community.

An understanding of dementia and the needs of those living with dementia and their carers would be an advantage. This experience could be paid, volunteering or lived experience.
