Job summary
ULHT's Frailty Service have an exciting opportunity for a Frailty Advanced Care Practitioner (ACP) to become part of our team at Pilgrim Hospital.
Main duties of the job
Successful candidates must be; a qualified ACP, be flexible, innovative, highly motivated and above all be committed to improving the care and assessment of frail older people in Lincolnshire.
Location: Pilgrim Hospital. The post holder will be identified a base site, but will be expected to work across all sites within ULHT to meet the needs of the service. There may also be a requirement to work as needed across partner organisations as part of the vision for an Integrated Frailty Workforce for Lincolnshire.
- Permanent position
  • The service currently runs Monday to Friday with a plan to extend to 7 days a week in the near future .
  • Hours 8.00 - 18.30
