ULHT's Frailty Service have an exciting opportunity for a Frailty Advanced Clinical Practitioner - Band 8a
**Location**: Pilgrim Hospital. The post holder will be identified this as their base site, but will be expected to work across all sites within ULHT to meet the needs of the service. There may also be a requirement to work as needed across partner organisations as part of the vision for an Integrated Frailty Workforce for Lincolnshire.
- Permanent position
- The service will eventually run 7 days a week. Hours 8.00 - 20.00
The Frailty ACP will use specialist knowledge and skills to provide autonomous assessments to patients. The ACP will be accountable for independent clinical assessment, diagnosis and treatment of patients who are frail as part of the Front Door Frailty Service.
The aim is to provide prompt specialist assessment to avoid hospital admissions where appropriate, to deliver a SDEC frailty model of practice and to promote care closer to home through our Frailty Virtual Ward
