About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our Home Visiting Team in Boston. As a key member of our team, you will be responsible for providing high-quality patient care in a bespoke area of primary care.

Main Responsibilities

Your duties will include assisting with daily home visits, triaging patients, and working on behalf of Boston Primary Care Network (PCN) practices. You will be employed and supported by Boston PCN Ltd and will work with a diverse, multi-skilled team of professionals.

Boston PCN covers six practices across Boston, Kirton, and Swineshead areas.
