About the Role
We are seeking an experienced Advanced Nurse Practitioner to join our Home Visiting Team in Boston. As a key member of our team, you will play a vital role in delivering high-quality patient care in a bespoke area of primary care.
Main Responsibilities
Your primary duties will include assisting with daily home visits, including visit triage and seeing patients for acute and minor illnesses. You will work closely with our Boston Primary Care Network (PCN) practices to provide comprehensive care to our patients.

Boston PCN covers six practices across Boston, Kirton, and Swineshead areas. Our vision is to create a community where everyone feels valued, has a sense of belonging, and can achieve their goals.
