Main Responsibilities- Provide expert clinical care to patients with complex needs, working collaboratively with multidisciplinary teams to ensure seamless care pathways.
- Develop and implement innovative care strategies to improve patient outcomes and experiences.
- Contribute to the development and delivery of education and training programs for healthcare professionals.
- Participate in research and audit projects to inform best practice and improve patient care.
- Work closely with colleagues to ensure effective communication and collaboration across teams.
- Current professional registration within the area of practice.
- Post-registration qualification or training relevant to the specialty.
- MSc in Advanced Clinical Practice or equivalent.
- Registered Independent Non-Medical Prescriber.
- Evidence of teaching, supervision, and assessment skills.
- MSc in Frailty or equivalent.
- Older Adults Fellowship (HEE).
- Mentor/assessor qualification.
- Management and leadership experience.
- Completed portfolio of supervised practice in Frailty.
