About this role

We are seeking an enthusiastic and motivated Advanced Nurse Practitioner to join our Home Visiting Team, working in and around Boston. You will be an experienced ANP who wishes to influence patient care in this bespoke area of primary care.

Main responsibilities

As a Home Visiting Professional, your main duties will be to assist daily with home visits including visit triage, seeing on-the-day patients for acute and minor illness, working on behalf of Boston Primary Care Network (PCN) practices.

Our Home Visiting Team is a diverse, multi-skilled, and experienced professional team of clinical and non-clinical staff. You will be employed and supported by Boston PCN Ltd.

Person specification

Essential qualifications

  • Registered with the NMC as RGN, with NMP qualification and evidence of masters-level education for advanced clinician practice or working towards, including advanced examination skills.
  • Experience of working within a demanding primary care clinical setting, preferably GP practice.
  • Independent prescriber.
  • Full and clean UK driving license and access to transport.
