Job Summary

United Lincolnshire Hospitals NHS Trust is seeking a highly skilled and experienced Advanced Care Practitioner to join our Frailty Service team at Pilgrim Hospital. As a key member of our team, you will be responsible for delivering high-quality patient care and contributing to the development of our service.

Main Responsibilities
  • Provide expert clinical care to patients with frailty, working closely with multidisciplinary teams to develop and implement individualized care plans.
  • Contribute to the development and implementation of service strategies, ensuring alignment with Trust objectives and national guidelines.
  • Collaborate with colleagues to identify and address areas for improvement, driving quality and efficiency initiatives.
  • Participate in research and audit projects, contributing to the advancement of knowledge and best practices in frailty care.
  • Support the development of staff, providing guidance and mentorship to ensure high-quality patient care and service delivery.
Requirements
  • Current Professional registration within area of practice.
  • Post registration qualification or training relevant to speciality.
  • MSc in Advanced Clinical Practice.
  • Registered Independent Non-Medical Prescriber.
  • Evidence of teaching, supervision and assessment skills.
Desirable Qualifications and Experience
  • MSc in Frailty.
  • Older Adults Fellowship (HEE).
  • Mentor/assessor qualification.
  • Management and leadership experience.
  • Completed portfolio of supervised practice in Frailty.
