We have an exciting opportunity for an Assistant Practitioner to join the integrated community diabetes service.


This role will work alongside the Integrated Community Diabetes Service and other Adult teams supporting the local population of Luton to improve diabetes care and reducing health inequalities.


The post holder will have a keen interest in the speciality of diabetes and preferably have experience of working with this cohort of patients.


The post holder will have or develop the skills to administer insulin and educate both patients and carers where applicable to self administer insulin.


They will work with a caseload of housebound patients who require their diabetes review and ensure the outcome is communicated to the relevant clinician.


The diabetes team provide education to their patients through home visits, structured education programmes and there will be a requirement for the post holder to participate in the delivery of this.


Assistant Practitioners (APS) are required to work autonomously and also as part of a wider clinical team, supporting registered Practitioners.


Work closely with all members of the multidisciplinary team, seeking opportunities to promote independent living, improvements in public health and contribute to reduction in inappropriate admissions to acute and long-term care settings.
