An Exciting opportunity to be part of a regional project supporting improved discharge processes from the Children's Hospital.

You will work alongside the multidisciplinary team organising and supporting discharge planning meetings to ensure safe and effective discharge of children, often with complex needs.

This role will involve working closely with families, often advocating for their needs and support and will involve developing close links with various outside agencies across the region.

This is a full time (minimum 30 hours) role which is currently funded for 1 year. You will be working within our Children's Hospital Operational team which includes both clinical and non clinical staff.


Southampton Children's Hospital is a large regional hospital with over 100 beds including 14 PICU beds and has close links to the neonatal unit.

It is situated on the edge of the New Forest within easy reach of London and the beautiful Dorset coastline.

YOUR RESPONSIBILITIES

WHAT YOU'LL DO


You will be responsible for identifying all long stay patients and that appropriate plans for care are in place and promoting early discharge planning, this includes maintaining a database to ensure that all members of the team can access the information.

You will attend the daily operational meetings and feed in to this meeting with discharge updates.


You liaise directly with all ward areas and discuss discharge planning and identify any potential barriers, allowing for early intervention.

Working closely with teams to ensure the IT systems regarding discharge planning are updated

Facilitate and attend multidisciplinary and discharge planning meetings and support the team to ensure any actions are completed.

WHAT WE'RE LOOKING FOR


We are looking for an experienced children's nurse with recent clinical experience with a sound understanding of acute, community, safeguarding and CAMHS care issues.


You will need to have excellent organisational and communication skills and be able to work both independently and within a team.


You need to be confident at meetings to advocate for the patient and family as well as the wider organisation to ensure safe discharge and on occasions chair the meetings.


This role is Monday to Friday with the aim for 12 hour cover on these day to ensure forward planning.

You should be flexible with your working pattern within these days.
