What you'll be responsible for:

  • To work clinically with service users in order to help prepare them for discharge from hospital. Working alongside the Pathways Team Leader, the inpatient wards, CRHTT, CMHT's and other agencies to support and plan clients timely discharge from hospital.
  • To support the planning, coordination, and transfer of service users across care pathways, liaising with Southern Health units and other NHS or private providers as required maximising the efficient use of inpatient services and transferring to other units as the client's presentation necessitates.
  • Alongside the Team Leader to provide support, guidance, and supervision to a small team of Band 4 Discharge Facilitators who work alongside Pathway Co-Ordinator to undertake administrative tasks to efficiently monitor clients discharge planning.
  • To assess, coordinate and broker the needs of services users, alongside CRHTT, Care Coordinators, CMHT's and other relevant agencies in partnership with the service user.
  • To aid the efficient and productive use of mental health services by undertaking tasks that release other clinical staff to focus on therapeutic care, whilst coordinating care pathway interface and transfer arrangements.

  • Experience in a similar role
  • Excellent communication skills (verbal and written)
  • Excellent organisation skills
  • Ability to meet deadlines and manage workload
