The focus of this role will be to lead our Collaborative Practice Services. This means you will be working with several volunteers (who are also patients), called Patient Champions.

This wonderful group of volunteers have formed a supportive, friendly, and fun team, which, under your leadership will continue to thrive, expand the services that they currently offer and create new offerings to all our patients across the PCN.

The activities on offer through our Patient Champions are currently monthly coffee mornings, book clubs, walking groups and IT for Health Groups, however they have much more to offer and need a motivated leader to support, encourage and embrace their wonderful ideas.


As part of this work, this will include (amongst other things): - Working with our Patient Champions on the planning and delivery of our monthly coffee mornings - Finding local venues to hold various groups and activities; managing bookings and ensuring payment of deposits and invoices - Supporting the various Patient Champion led groups to enable them to plan and deliver the monthly offerings (Book club, IT for Health group and Walking group) - Hosting monthly meetings (either in person or online) with all of our Patient Champions to track progress, motivate the team and to offer any support needed - Supporting the creation of new groups as suggested by Patient Champions - Recruiting new volunteers to join our Patient Champion team - Developing, encouraging and creating new innovative ideas to expand our Patient Champion led services - Ensuring the team is connected between themselves and with our PCN and Practices - Bringing the team together for social activities to maintain and build relationships - Tracking the impact of our services and measuring our success Separately to working with our Patient Champions, this role will also involve the coordination of our Urgent Care Clinics (UCC).

The UCC is our collaborative service across all our local GP Surgeries to deliver daily urgent care and enhanced services to our patients.

The fundamental principle of this part of the role will be to navigate patients through care resource options to ensure the right patients see the right health care professional at the right time.


This is based at Hythe Hospital which is a fun, vibrant and busy hub with lots going on As part of this work, you may be (among other things): - Providing admin support - Helping with weekly rotas for our Practitioners - Liaising with our Partner Teams who are also based at Hythe Hospital such as Southern Health Foundation Trust - Tackling and trouble shooting for all sorts of issues as and when they arise.

For example, you could be arranging for someone to fix a broken front door one minute and helping with a patient query the next Beyond these two main parts of the role, you will have the opportunity to get involved in a range of different projects, balancing the needs of the business with your personal likes and interests.

For example, we might ask you to support with:

  • planning our seasonal Covid/Flu vaccination clinics
  • working with patients with long term health conditions to help them to access appropriate resources
  • supporting new initiatives as they arise (such as our recent menopause workshops or diabetes workshops)
  • proactively identifying and engaging with patients who may experience inequality to health care to ensure they are supported and signposted accordingly
  • working on regulatory requirements to ensure we are complaint with our Care Quality Commission (CQC) obligations
  • as well as forging new projects based on your own passions which support our work as a PCN
