The surgical discharge lounge operates from 10:
Monday to Friday.

We are looking for a pro-active, engaged and committed individual to run our surgical discharge lounge.

The post holder will be responsible for providing the ward teams with support when planning for and discharging our patients.

You will be required to work closely with the multi-disciplinary team to achieve a safe, efficient and timely discharge of our patients.

This role is supported by the care group management team (CGMT).

YOUR RESPONSIBILITIES

WHAT YOU'LL DO


The Associate Practitioner will work collaboratively as part of the multi-disciplinary team within the surgical wards to provide a seamless service & high quality care to patients.

This role is vital to ensuring the safe and effective discharge of our surgical patients. And also in our vision for improving the flow of our surgical in-patients.

WHAT WE'RE LOOKING FOR


As a care group we have a total of 149 beds, inclusive of admission areas that support many surgical specialities.

As a team we are proud of our record in supporting emergency flow for the organisation making sure that patients see speciality surgeons as soon as possible so those patients get the timely care they need.


As a speciality we have a 12 trolley acute assessment unit which flows into our 30 bedded acute admission area, an ENT treatment room where patients are admitted from the emergency department, clinic areas and other regional hospitals.


As the successful post holder you will:

  • be required to at all times maintain the dignity and respect of patients
- you will need to be visible and approachable
- effective communication with the multi-disciplinary team will be essential

  • You will be required to attend the wards and interact with the senior nursing teams to identify suitable patients to come to the discharge lounge.
- give and receive regular reports on the progress of patients
- participate in the dispensing and checking of medications
- you will be required to book and monitor hospital transport
- assist nursing teams in referring patients for on-going community nursing support
