An exciting secondment opportunity has arisen for an experienced and motivated registered nurse to join our friendly and progressive Continuing Healthcare (CHC) team on a one year secondment.

Main duties of the job

We are looking for an experienced individual, with excellent communication and interpersonal skills and experience in working at a senior level within NHS Continuing Healthcare. You will support the CHC Heads of Service to deliver on all the areas of CHC, Funded Nurse Care (FNC) and Personal Health Budgets (PHBs), previous knowledge and experience in CHC/FNC is essential.

The post holder will work as a member of the CHC service within the Nursing and Quality division of the ICB and will be responsible for ensuring the CHC service is provided to: -

Undertake all requirements in relation to the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (July 2022) Ensure compliance for the service within all other legislative frameworks Implement agreed strategies and plans that provide assurance and quality in relation to the Continuing Healthcare National Framework and all associated guidance and polices as directed by the Department of The post holder will provide clinical leadership and management to the clinical team leaders and wider CHC team.

Job responsibilities

Clinical Responsibilities

Assist in the development of a clear strategic plan for CHC, that ensures compliance with all aspects of the CHC National Framework and carry out agreed plans and processes developed by NHS Herefordshire and Worcestershire ICB.

Implement agreed staffing strategies which facilitate an effective and flexible workforce able to respond to service demands, corporate and national and ICB provide effective clinical leadership to clinical team leaders & CHC team members, enabling the team to deliver to the highest possible standards, ensuring that evidence based practice is used when completing assessments and commissioning individual care packages.

Working in accordance with the CHC Operational Policy, to ensure completion of referrals and reviews.

To monitor effectiveness of internal ways of working and develop action plans as required for improvement/gaps in system effectiveness. The system will cover:

Ensure timely assessments and reviews are undertaken by members of the team to determine the eligibility of CHC funding in accordance with the National Framework.

The management of all eligible cases, ensuring systems are in place foreffective reviews.

Ensure timely reviews are taken by appropriate clinical staff in relation to out of area CHC placements

To prioritise referrals as required

Ensure that eligible cases have a commissioning care plan in place.

Overseeing clinical placement of individual patients with specificcomplex needs requiring named-patient packages.

Review complex cases to ensure that plans are clinically appropriate to meet the assessed need of the individual through the provision of high quality, value for money packages of care. Agree financial expenditure for these cases on behalf of the ICB within delegated limits.

Use expert clinical knowledge to verify adult continuing healthcare recommendations.

Work in conjunction with the Business Manager in regard to the management of productivity for the core service Understand the reporting system for CHC for new referrals and reviews including all care packages agreed with the ICB.

Ensure timely responses and assessments of all requests for appeals and retrospective reviews in line with the Department of Health directions.

Assist the Heads of CHC in the development, implementation and monitoring of a clinical plans for the service within the area of responsibility.

Oversee specific projects in relation to CHC, in conjunction with the Heads of CHC.

To be responsible for the co-ordination of continuing healthcare panels that support clinical decision making.

To maintain appropriate records within the team and communicateinformation as required with all relevant parties whilst maintaining appropriate confidentiality of patient data.

Liaison and performance reporting to and on behalf of the ICB with other statutory bodies and regulators including NHS England, the Care Quality Commission, Health Overview and Scrutiny Committees and other relevant bodies, where appropriate.

To be knowledgeable and understanding the Safeguarding Adults, Deprivation of Liberties and Mental Capacity Act arrangements and ensure processes are in place to identify and deal with infringements of these policies in line with Herefordshire and Worcestershire ICB policies.

To lead on the development of this role across health & social care within all care groups in order that consistency of application of continuing healthcare is applied.

To develop and maintain good working relationships with stakeholders across organisational structures, to maximise opportunities for modernisation and integration.

To lead in complaint investigations as directed by the Heads of CHC.

To lead in clinical audit relevant to the service ensuring evidence based practices are adopted.

Apply, and adhere to, the ICBs Choice, Resource and Allocation Framework for individual cases.

Create and implement learning and development programmes to ensure nursing staff are appropriately equipped for high quality professional practice within their CHC.
