Job summary

Assistant Practitioner Therapist (band 4) Islington Community Mental Health Team

Assertive Outreach model works flexibly and creatively using team approach to engage service users in ways and places that acceptable to them. Staff engages more closely while allowing enough room for autonomy for service users

Our objectives are to:

Use team approach to engage service users more assertively to promote recovery and reduce number of readmissions to hospital Use a Recovery orientated approach in all our work with service users and carers. To respect service users as experts by experience To always treat service users with dignity and respect To engage service users collaboratively through Dialogue + care planning To agree individual, tailored care plans with service users and their carers To provide support to carers so that they can continue to support their relative. Transfer back the service user users to recovery teams and other community mental health teams once good engagement has been established.

Main duties of the job

We are looking to recruit an Assistant Practitioner who shares our passion to strive towards excellence in people's mental health and social care and who affiliates with our Trust's culture pillars where staff and service users feel safe, valued and are empowered.

As an assistant practitioner you will be responsible for supporting the team to deliver a range of health and/or social care interventions. You will have a small caseload of patients who require low-level or preventative interventions. You will take a lead on supporting carers, which will include carrying out an assessment under the Care Act. You will be supported by a multidisciplinary team.

We are a busy and dynamic service and caseloads are reflective of this. This requires you to have good organisational skills and an ability to work flexibly as part of the team. The role involves visiting patients at home and you therefore must be able to get around Islington using Public Transport and/or walking/cycling.

You'll be able to demonstrate experience in working with adults' mental health care and have good communication and interpersonal skills. You should have some understanding of the wider health and social care system. Being computer literate is a must and you have to be able to use Word, Excel and other relevant IT systems.

This may mean that you are required to work at any location that fall under Barnet, Enfield and Harringay Trust and Camden and Islington Trust. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.

Person Specification

Education/Qualification

Essential

Diploma in Mental Health Practice or NVQ level 3 in Mental Health with post qualification evidences of further study / additional training to diploma level or NVQ 4 in Mental Health or equivalent level experience. or Evidence of extensive and current paid experience of working with service users who have severe and enduring mental disorder/illness. Educated to GCSE level or equivalent (with a minimum of 5 A - E qualifications - which must include Maths and English).

Desirable

To have experience of or if appropriate to service needs be willing to undertake training in de-escalation or breakaway techniques.

Experience/Knowledge

Essential

Experience of caring for people with severe and enduring mental illness and or dementia Previous experience of working in a Health or Social Care setting in a paid or voluntary position Demonstrate an understanding of the key sections of the Mental Capacity Act and Care Act and its use in practice. Demonstrate a sound understanding of the needs and difficulties of older people and their family carers. To demonstrate an understanding and/or practical experience of delivering practical support to older adults with mental illness and/or dementia in the community. Knowledge of the comorbidities that maybe present as a result of older age and increasing frailty.

Skills/Abilities

Essential

Excellent verbal and written communication skill. Able to work on own initiative. Able to work as part of a team Motivated and enthusiastic about working with older people with mental illness and/or dementia. Competent using computers and keyboards for entering personally generated data, writing and responding to emails, being able to create and manage spreadsheets and date reports, conducting web searches etc.
