The HTT works intensively with people with dementia and people with acute metal illnesses and age related frailty. The service is looking for a Band 5 clinical practitioner who is passionate about working with older people experiencing a mental health crisis and those who have dementia.

The service has an intense interface with psychiatric liaison services and inpatient services, providing gatekeeping assessments authorising admissions as well as facilitating early discharges making this an ideal opportunity for an enthusiastic practitioner who wants to work in a well-established team and influence how care is delivered for this client group.

The post holder will work as an independent practitioner but within a supportive team to deliver care that enables service users to remain in their own homes during a period of mental ill health. Training opportunities will also be provided to enhance your knowledge around dementia and age related frailty.

Main duties of the job The post is open to B5 Registered Mental Health Nurses including newly qualified staff and non-registered clinicians who can demonstrate suitable experience in working with service users experiencing a mental health crisis. We are looking for skilled and dynamic clinicians with a passion for delivering outstanding patient care.

Successful candidates will work alongside the Band 6 clinicians in the team to deliver high quality psychosocial assessments and interventions with older adults.

As a clinical practitioner you will be supported in completing assessments with a more senior clinician, working within an MDT model to discuss complex cases. As a new member of the team, you will receive a thorough induction to the service, monthly clinical supervision and regular reflective practice sessions.

Part of the Clinical Practitioner's role is ensuring that service users are supported through the initial stage of a crisis with high quality trauma informed therapeutic care and delivering effective care and interventions.

The service acknowledges that we often see people at their most vulnerable and as such it is essential that we provide, compassionate and trauma informed care. The service is dynamic and fast paced and the role therefore requires stamina, resilience, and the ability to work well with colleagues. We currently deliver the majority of our care to residents in the London Boroughs of Camden and Islington. For more information, please access the following link: Job description Job responsibilities Reporting to the Senior Clinician or designated supervisor the post-holder will undertake a range of necessary tasks, including taking and screening referrals, prioritising referrals, identifying appropriate action in processing referrals, carrying out assessments, developing and agreeing care plans with service users, implementing and evaluating interventions, and identifying and referring on to appropriate support services for service users ongoing care. This will involve delivering care and interventions appropriate to the post holders level of experience and competences without direct supervision.
  • To contribute to the multi-disciplinary team comprehensive, culturally appropriate assessments to establish eligibility for health and social care services.
  • To undertake continuous risk assessment and to contribute to effective risk management. At all times to act in accordance with trust policies in relation to risk assessment
  • Recognising and valuing service users as individuals the post holder will ensure that the care provided respects equality and diversity that is supportive of service users and their carers. This will include encouraging service users to accept an optimum level of responsibility for their individual programme of care and with their consent, where appropriate, seek the cooperation of friends, relatives and/or carers.
  • Utilising agreed frameworks and guidance such as NICE or NSF evidence- based best practice and or local policy and procedure, as well as a range of profession specific tools, the post holder will actively assess, plan and determine the care needs of service users. They will undertake and contribute to specific or multi-disciplinary team reviews of such care.
  • To undertake the physical health care assessment and or on-going physical health monitoring / review of designated service users, recording any changes in the service users physical health, reporting these to appropriate members of the team and acting appropriately on the advice given.
  • To identify and provide information to service users that promotes their health and wellbeing. Directing them as appropriate to others / other services in order to address any identified needs.
  • The post holder will lead in identifying support and help for service users to become aware of their own physical health and well being and engage service users in addressing these needs, for example smoking cessation and obesity.
  • This role will also involve the post holder providing appropriate information and support for other staff.
  • To contribute to the ongoing quality of service and care, identifying and promptly reporting any risk issues or other significant factors in relation to service users health and social care.
  • The post holder will participate in and contribute to a range of service-led, commissioner-driven or nationally directed monitoring or performance management, related to care, care delivery or service provision.
  • Recognise and respond appropriately to any challenging behaviour presented by service users or others.
  • To undertake and assist in the organisation of the day-to-day smooth running of the service, reporting any issues relating to the fabric or condition of the work environment.

  • The post holder will participate in the ongoing monitoring of service users medication regarding efficacy and possible side effects, reporting concerns promptly to identified members of the nursing staff or medical staff within the team. The post holder will also support clozapine monitoring for service users who are prescribed this.
  • To follow trust policy in promptly reporting all accidents and incidents and to contribute to undertaking complaints or investigations if required.

  • Contribute to ensuring effective communication with service users, their carers, family, friends, external agencies, and staff colleagues. Always acting in a dignified and responsible manner with service users, visitors, carers and colleagues listening carefully and responding using appropriate language and communication skills which acknowledge cultural differences and professional boundaries.
