We are seeking an experienced Clinical Lead Practitioner to join our team at King's College Hospital. The successful candidate will be responsible for leading a team of Frailty Practitioners in the delivery of high-quality care to patients with frailty.
Key Responsibilities:- Lead a team of Frailty Practitioners in the delivery of the King's Frailty service.
- Develop and implement clinical protocols and strategies to enhance care.
- Coordinate a seamless service through the development of enhanced interdisciplinary team processes and communication.
- Provide expert advice and support to older people and people with frailty, their families, and other healthcare professionals.
- Manage a patient caseload and make and receive referrals.
- Professional registration - Nurse or Physiotherapist or Occupational Therapist.
- Minimum undergraduate degree relevant to profession.
- Post-graduate degree relevant to specialist area, or willing to work towards.
- Experience in managing a clinical team and relevant, demonstrable experience in specialty area.
- Excellent communication skills both oral and written.
In carrying out the duties of the post, the employee is required to work by the policies and procedures of King's College Hospital, including Health and Safety, confidentiality, data protection, and equal opportunities.
