Health Engagement Worker

Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Health Engagement Worker within the Early Intervention Children's Centres to work full time based in Halton.

The salary for this permanent Social Worker job is up to £28,666 per annum.

This role is an exciting opportunity to join an innovative project within a small team.

The role acts as a link with GP surgeries, helping to support children, young people and their families to access community and specialist services to meet any non-medical needs by signposting / referring young people and their families.

As well as helping families and improving patient experience, you will act as a resource to support the GP practices, offering advice and guidance to practice staff.


The service forms part of Halton's Early Intervention Strategy which means there will be better outcomes for children and families.

Effective Early Intervention will resolve problems before they become overwhelming and require high cost, reactive services. The service works towards implementing this strategy within GP surgeries.

The role gives you the opportunity to contribute towards developing a service that is unique to Halton and has potential for growth.


In addition, you will also be working alongside GP practices ensuring that health staff are aware of what is available to support families locally and how to access this.

We aim to strengthen links between GP practices and external services.

Main duties:

  • Provide support to several children and families, working in partnership with them and other relevant professionals to identify needs and coordinate appropriate support plans
  • To provide information, advice and signposting to parents/ carers and children/ young people so they can fully access all available services to meet their needs
  • Liaise and communicate with other agencies, parents and professional staff so that appropriate resources may be identified and mobilised for the benefit of children, young people and their families
  • Maintain effective and accurate record systems to enable the preparation of necessary reports to contribute to the service planning process
  • Where appropriate, offer advice and support to other staff, to ensure the effective and efficient organisation of the service
  • To work closely with GP practices to develop and promote access to services
  • Be aware of and comply with the statutory requirements, group policies, equal opportunities and other regulations and procedures
- especially in respect of safeguarding
- bringing any safeguarding concerns to the attention of the Health Engagement Officer/ Principal Manager

  • Monitor and continually review the service user needs, and make recommendations for improvement to contribute to the achievement of the overall service objectives

Requirements of this job:

  • NVQ Level 3 in children & young people/health or social care or related field
  • Experience of working with children and families in early help, or related environment
  • Experience of working collaboratively with colleagues from other organisations, including social care, health, education, voluntary sector
  • Experience of using information technology/systems for recording direct work
  • Continual professional development
