The RoleWe are seeking a dedicated and compassionate Support Worker to join our team. As a Support Worker, you will play a vital role in supporting individuals with complex needs, promoting their independence, and enhancing their overall quality of life.
Key Responsibilities- Provide person-centered care and support to individuals with diverse needs
- Assist with daily living tasks, such as personal care, medication management, and meal preparation
- Encourage and support individuals to participate in activities that promote their physical and emotional well-being
- Work collaboratively with colleagues and other healthcare professionals to ensure seamless care delivery
- Contribute to the development and implementation of individual care plans
