We are looking for an experienced and highly motivated Cardiology Nurse with a keen interest in heart failure to join the heart failure team at North Middlesex University Hospital NHS Trust.

You will join a dynamic team, caring for patients across the in-patient/outpatient pathway supporting patients within the hospital setting and to transition home ensuring seamless pathway of care.

We are looking for an enthusiastic, pro-active, self-motivated nurse to join the team who is comfortable working autonomously and within the wider multidisciplinary team particular with excellent communication skills. You will also value team player qualities and who is highly organized and experienced in auditing of processes to support our patients gain a positive experience.

Main duties of the job

Duties will include: assessing patients acutely admitted within the Trust, as well as acting as an autonomous practitioner in outpatient clinics supporting patients and families referred to our service providing communication, counseling, smooth running of clinics and linking the pathway of care amongst different parts of the team, the hospital and community, helping to ensure a seamless pathway of care. You will be expected to support patients to engage and uptake the cardiac rehabilitation programme. Part of the role will include submitting data for the National Heart failure Audit.

Detailed job description and main responsibilities

The post holder will have the responsibility for assessment; planning, development, implementation and evaluation of individual programs of client care within Adult Heart Failure Service. You will manage the health care of complex and vulnerable individuals with diagnosed heart failure (NYHA 1-IV) referred to the service.

You will assist and deliver in the coordination of the inpatient heart failure INreach service, participating in heart failure consultant rounds and fulfilling the National Heart Failure Audit requirements. You will autonomously be expected to review patients with heart failure admitted to North Middlesex Hospital NHS Trust (NMUH) using enhanced clinical skills following an appropriate referral and/or through actively monitoring for admissions of patients with suspected heart failure.

A key aspect of role is re-admission prevention management including reviewing patients within 2 weeks of discharge from hospital; undertaking comprehensive assessment, physical examination and appropriately treat patients; referring when necessary to other health care professionals or services.

You will screen patients who are at risk of deterioration and early signs of heart failure decompensation and respond to changes in patient's clinical status appropriately.

You will manage own caseload in clinics delivering individualised care, initiating and titrating medications and facilitating lifestyle changes through education and support to patients, carers and other health professionals.

You will be expected to be involved in the development and sustaining of cardiac rehabilitation service for heart failure patients.

You will utilise expertise in research based heart failure practice, use critical appraisal skills to expand and inform daily clinical practice.

You will be expected to routinely undertake independent Nurse Prescribing while complying with NMC code of Conduct to undertake responsibility for prescribing with locally agreed protocols and national guidelines.

You will support the Lead nurse to set, monitor and evaluate standards of heart failure practice to ensure quality and effective governance for the service in accordance with NICE guidance and assist

in the on-going development and innovation of the NMUH Heart Failure Service as dictated by demand and changes in service delivery.

Essential criteria

Registered Nurse (RGN) Non-Medical Prescribing Qualification - (V300) or willing to undertake Clinical examination /assessment qualification or willing to undertake Evidence of on-going training and development.

Desirable criteria

-Post graduate qualification in Heart Failure or related field (willing to complete)

Knowledge/experience

Essential criteria

Extensive, in-depth specialist knowledge and experience within heart failure management Proven knowledge of Heart Failure National and Local Guidance (NICE guidelines and DOH). Effective written and oral communication and liaison skills

Desirable criteria

Presentation skills/ability to deliver training Experience in palliative Care
