Job summary

People living in care homes should expect the same level of support as those living in their own homes. The national framework for the Enhanced Health in Care Homes (EHCH) model of care provides steps to achieve this. We are moving away from traditional reactive models of care delivery, to proactive care, that is centred on the needs of individual residents, their families and care home staff.

The Advanced Clinical Practitioner (ACP) will be responsible for the day-to-daymanagement of staff and service delivery within the EHCH team. The ACP willwork closely with the Primary Care Network clinical leads and the Operational Delivery Group todrive the implementation and development of EHCH framework across Southport andFormby. Their role will also include business planning, performance, andfinancial management, and supporting the management of day-to-day serviceproblems.

Although the post is advertised as 30 hours part-time, we will happily accept applications from those individuals who are looking for full-time hours.

Main duties of the job

We wish to recruit an Advanced Clinical Practitioner with advanced skills to be responsiblefor the day-to-day management of the clinical team including a trainee ANP, SpecialistFrailty Nurses, and Nursing Associate, with support from the Advanced PharmacistPractitioner, Primary Care Network clinical leads and the Human ResourcesManager.

Theclinical role will require the postholder to have extensive experience in assessingthe needs of older people, including experience of the holistic comprehensivegeriatric assessment and be able to workindependently within the Care Home environment. The postholder will requireadvanced communication skills to discuss and complete proactive individualisedclinical management plans for Care Home residents. The desired outcomes fromassessments include improvingthe health experience of Care Home residents, havinga positive impact on primary care service provision, improvinginterprofessional relationships, and reducingunnecessary and potentially harmful hospital admissions.

Job responsibilities

The postholder will require a Masters degree inAdvanced Clinical Practice.This position willbe a varied role, with management responsibilities for the team, clinicalleadership, clinical development of the team and clinical reviews with CareHome residents.

In terms of management responsibilities, the postholder willbe responsible for the day-to-day management of the clinical team including atrainee ACP, Specialist Frailty Nurses, and Nursing Associate, with supportfrom Advanced Pharmacist Practitioner, the Primary Care Network Clinical Director and theHuman Resources Manager. This willinclude 1:1s, team meetings, any new starter processes, probationary reviews, andcarrying out annual appraisal for existing clinicians. The postholder will also work closely withthe Primary Care Network (PCN) leads to monitor deliveryagainst the requirements of the EHCH specification and identify opportunitiesfor improvement.

Theclinical role will require the postholder to have extensive experience in assessingthe needs of older people, including experience of the holistic comprehensivegeriatric assessment and be able to workindependently within the Care Home environment. The postholder will requireadvanced communication skills to discuss and complete proactive individualisedclinical management plans for Care Home residents. The postholder will communicate individual management plans with systempartners and developrelationships with the PCNGP practices, Care Homes, community teams, mental health services, palliativecare services, and social services, to enable residents to meet their health goalsand be cared for in their preferred place of care.

Thepostholder will need to be confident with Multidisciplinary working style andutilising the current Multidisciplinary Team processes in place toappropriately discuss a residents care. The role involves clinical discussionswith other members of the team and taking responsibility for thedecision-making during these discussions.

Thepostholder will be expected to bring their wealth of experience to enhance teamworking and ensure the team continues to grow.The postholder will understand the need to work closely with PCN Leadsto ensure plans are in place to meet demand within the set KPIs within the PCNcontract.
